There will be a flu shot clinic on October 4, 2011 from 12-6PM at the Cattaraugus County Health Department in Salamanca, NY.
BRING your insurance card - we will bill the following: Medicare, Blue Cross/ Blue Shield, Independent Health, Univera, Fidelis, Today's Options, Evercare, and Humana
NO insurance? Cost = $20.00
